Just a thought if you have continuing issues...
I sometimes pop the beans in Jiffies that have been hydrated with weak nutrients and pot them once the root emerges. This way there are no bugs and it's easier to fit them in a humidity dome if required. I had a few stall in soil and it turned out there was something gnawing on the roots.
The icebox method consistently works well. Fill it about 1/4 with water heated by an aquarium heater and the jiffies float in a plastic container without a lid. Keep the icebox closed until you see some activity then you can leave it open until you see a tap root then pot
