If it were me I would call the Court. You don't want to fool around with an outstanding warrant at the worst possible time. Explain to them what happened and tell them you are permanently moving. Pay the online fee and show them:

I've been told by reliable first hand knowledge that those change of address cards that are "provided" are summarily discarded by postal employees. It takes considerable time away from their "regular" duties. Pay the $1.10 fee.
