I grew professionally in Colorado and they usually asked for a business ID. In Michigan, it isn't necessary, but helps prove you are receiving "donations", paying your taxes and that you are a non-for-profit organization.
As for anonymity, obtain a free tax ID from the social security office under a false name and use that to get the license, or list your company as medical instead of horticultural on the form.
