Weird dried leaves and slimy root (but no brown root?!?)

This is my seedling I sprouted on the 14 of October. I started to notice there are some yellow spot on the lowest fan leaf and it spreaded all over the fan leaf in two days. Other new fan leaves started to have the same problem. At first I thought it was nutrition burn so I change it to a new reservoir with less amount of nutrition. It seems to stop the weird dry patches to grow but the plant wlit and the root looks a bit slimy than before. I use two large 5' air stone to provide oxygen to the root. I was growing this plant with two other plants in the same reservoir before I moved it to a new reservoir. Can anyone help pls I'm so worry about her

Changing all the water and adding a little hydrogen peroxide may help with that slime stuff.

BTW: Next time don't upload pics with pretty fingerprints. You may want to edit that later on.

Sorry Im new . BTW do u know what would be the causes for the dry and crispy leaf??
