Need help, she's dying

Under 250 hps, lamp is 20cm from top, water once in week because i got vermiculite in my soil.I also give her bloom fertilizer once in 2 weeks and seaware fertilizer the other time i water the plant. So whats wrong . I Thought heat stress or N-deficity but wanted to ask experts.

After looking at your photo album of the plant I would guess it is related to your nutrients. The leaves in some pictures have burned leaf tips and some of the leaves have lost some of their green color. You could try just re-potting or transplanting this plant into another pot with fresh soil. It would be safer then giving it more fertilizer/nutrients.

Also don't forget to check both the PH of the soil and relative humidity levels, RO of around 60% is good.

I think Pinkladyeater is right that a repot might be in order, even though they've been flowering some time. I think you could water more often (can't remember the last time I advised that!) because that plant is pretty large for the container. It probably has roots all over the bottom, so I'd even be comfortable letting it sit in a tray of it's own runoff as long as it could absorb it in in a few hours.
 
pH testers are cheap and worth it! i'd recommend getting one. It doesnt look all that serious from the pictures, you probably caught it early. How far along is she?

Try a larger pot with some fresh soil which will act as a buffer incase the problem is fertilizer build up or a nutrient mineral deficiency of some kind.
