Where is the infirmary sick plants checklist?

Someone make a sticky of it and include the form, too many people post problems without including any information.

there use to be a list around of DEF that let you figuare tings out yourself, it was like IF THE PROBLEM STARTED AT THE TOP AND WENT DOWN IT COULD BE THIS--or IF THE PROBLEM STARTED FROM THE MIDDLE AND IS MOVEING UPWARD IT COULD BE THIS...

there was like 20 different things on that list and it was very helpful indeed, i had it copyed at one point but have sence lost the printed papper i had made...

if any one remembers this LIST PLEASE give a link to it if ya can..
