SSD has never messed with me, as far as paper work goes it depends on if your doctor will sign for you or if you have to goto a clinic. If your doctor will sign all you need is the application packet and be sure to keep copys and send registered mail, this counts as your card until they send the real one. If you have to goto a clinic you will need to get 3 vists worth of chart notes that are less than 3 years old, you then take those to the clinic and they will make sure you qualify and sign your app.
